Breast Augmentation at Maletic Clinic — Zagreb

Implants, your own fat tissue, or a hybrid method - a natural result, a safe plan, and clearly guided consultations

Breast augmentation is not a “one procedure for everyone” – it is individual planning that combines anatomy, aesthetics, and safety. Some women want a subtle increase in volume, others want fuller cleavage, and others want to restore shape after pregnancy or weight loss.

At Maletic Clinic, every breast augmentation plan begins with a precise assessment of body structure, body proportions, and realistic expectations. The result must be harmonious with your body – and stable over the long term.

WHAT THE PROCEDURE IS

What is breast augmentation and what can it achieve?

Augmentation mammoplasty is a surgical procedure that increases breast volume and improves breast shape. It is most often performed with silicone implants, transfer of the patient's own fat tissue (lipofilling), or a combination of both methods. The goal is to achieve harmonious volume, stable positioning, and a natural transition in relation to the patient's body structure.

When is the procedure the best solution?

  • after puberty, the breasts did not develop to the desired volume
  • after pregnancy and breastfeeding, the breasts lost fullness and firmness
  • after weight loss, volume decreased and proportions changed
  • there is breast asymmetry that the patient wants to correct
  • the goal is to balance body proportions and feel more confident
Aesthetic philosophy

Natural result

The goal of well-planned augmentation is not "larger breasts at any cost".

The real focus is on harmonious volume and a natural transition that respects your anatomy.

PROCEDURE METHODS

Breast augmentation methods - how to choose the best option for your body?

The real question is not “implants or fat tissue?” – but “what is right for my body and my goal?”. The answer does not come from preference, but from analysis.

Method 1

Implants

The most predictable method when the goal is a clear increase in volume and shaping of the neckline. Precise control of size and form.

  • Predictable and long-lasting result
  • MENTOR™ - lifetime warranty
  • Selection according to body measurements
Method 2

Lipofilling

Ideal for a more subtle result without implants. Liposuction from a donor area and careful layered injection of fat.

  • Natural breast feel
  • Shaping of the donor area
  • Usually 2 sessions (3-month interval)
Method 3

Hybrid method

A combined approach: implant volume + the natural softening effect of your own fat. For correction of asymmetries.

  • Softer implant transition
  • Precise contour correction
  • Combines the advantages of both methods

Comparison of the three breast augmentation methods:

Method Ideal candidate Advantages Note
Implants Clear increase in volume and neckline fullness Predictable result, precise projection control Selection according to chest measurements
Lipofilling Subtle result • sufficient fat tissue reserve Natural feel, shaping of the donor area Usually 2 sessions • 3-4 months apart
Hybrid method Slim patients • asymmetries • softening effect Predictable volume + natural transition Combination of implants and lipofilling
MENTOR™ implantati - Johnson & Johnson

For breast augmentation, we use only MENTOR™ implants - a global leader in breast aesthetics with a tradition of more than 50 years and CE and FDA certifications. Every patient receives a certificate for the implanted devices - a document stating their type, series, and origin.

TECHNICAL PREPARATION

The most important decisions before surgery: implant shape, incision, and placement

Every breast augmentation includes three key technical decisions that directly affect appearance, feel, recovery, and the long-term stability of the result.

Implant shape - round or anatomical?

The decision is never just an aesthetic preference. It depends on your starting anatomy, the amount of tissue, chest width, and what goal is realistic for your body.

Comparison of implant shapes:

Shape Characteristics Typical indication
Round Emphasized fullness of the upper breast pole, a more defined cleavage Goal: visible, more pronounced cleavage
Anatomical ("teardrop") A more natural transition, with an appearance that follows breast anatomy Goal: discreet, natural appearance

Incision site - where will the scar remain?

Most often, we plan inframamarni rez – an incision in the natural fold beneath the breast. This technique gives the surgeon excellent control and precision, while the scar remains hidden in the fold. An inframammary incision provides good control and precision, and the scar remains hidden in the fold beneath the breast. Alternatively, the incision may be along the edge of the areola (periareolarno) or in the armpit, but each option has specific features and is not equally applicable to every anatomy.

Where is the implant placed - above the muscle, below the muscle, or below the fascia?

Implant placement directly affects appearance, feel, the risk of visible implant edges (rippling), and recovery time.

Implant placement - comparison of the options we use at Maletic Clinic:

Placement How it works Advantages Best suited for
Above the muscle (subglandularno) The implant is placed beneath the breast glandular tissue and above the pectoral muscle Shorter recovery, less discomfort, and the implant does not move when the muscle contracts Patients with enough of their own tissue to provide adequate implant coverage
Below the muscle (submuskularno) The implant is placed beneath the pectoralis major muscle Better upper-pole coverage, lower risk of visible edges, and a more natural transition Slimmer patients with thinner subcutaneous tissue
Below the muscle fascia (subfascijalno) The implant is placed above the muscle but beneath the fascia - the connective sheath covering the muscle Easier recovery compared with submuscular placement, with an additional layer covering the implant edge Patients who want a compromise between recovery and coverage
Dual plane The upper part of the implant is beneath the muscle, while the lower part is beneath the glandular tissue Natural appearance, good upper-pole coverage, and lower-pole fullness The most commonly used technique - suitable for a wide range of anatomies

Implant placement directly affects appearance, feel, recovery, and the long-term stability of the result. At Maletic Clinic, we choose placement according to the medical indication for each patient - not according to preference.

In patients with thinner tissue, placement below the muscle or dual plane is often considered. Every placement has advantages and limitations – individual assessment for each patient is essential.

CONSULTATIONS

Breast augmentation consultation - what do assessment and planning look like?

Finally - a surgeon who analyzes your anatomy before recommending an implant.

A high-quality consultation is not a “size offer” – it is a medical examination and planning tailored specifically to you. During the examination, we assess chest width, symmetry, nipple position, skin quality, the amount of existing tissue, your goals, and lifestyle.

If you have a specific wish – “natural and discreet” or “more pronounced cleavage” – the purpose of the consultation is to translate that wish into a realistic, safe plan. And to clearly explain what is feasible without compromising tissue or long-term stability.

During the consultation, the patient always tries on a special bra into which we place different implant sizes – giving an immediate impression of the size and weight of each implant in the context of her own body.

After the doctor’s analysis of proportions and dimensions, the decision on size and shape is made together – in a discussion between the surgeon and the patient. No general “one size fits all” approach.

Expert consultation

During the consultation, you receive:

  • Medical assessment of your anatomy
  • Comparison of all three augmentation methods
  • Trial implant sizing in a bra
  • Individual treatment and recovery plan
  • Realistic expectations and clear answers

CONSULTATIONS

Breast augmentation consultation - what do assessment and planning look like?

1998. 25+ years of experience
MENTOR™ Certified J&J implants
CE + FDA International certificates
24h Medical care

Breast augmentation is not only a technical surgical procedure – it is a combination of surgical precision, understanding body proportions, and long-term result planning.

In breast augmentation, the difference between an average and an excellent outcome is usually not visible in the first week, but several years after surgery. That is why the surgeon’s experience, knowledge of all techniques, and proper indication assessment are crucial.

Even if at other consultations you received only a standard offer without a detailed analysis of chest width, base width, and tissue quality, at Maletic Clinic we take a different approach.

Maletic Clinic has been performing breast augmentation surgery since 1998. In addition to implant augmentation, we also perform fat transfer, breast lift, and breast reduction. Thanks to our knowledge of all techniques, we individually choose the approach that is optimal for each patient.

  1. 1
    Individual analysis of body proportions

Implant size and shape are selected according to chest width, tissue thickness, skin quality, and long-term tissue load

  2. 2
    Precise planning of implant placement

Subglandular, submuscular, or subfascial - a medical indication, not an aesthetic preference

  3. 3
    Individual implant selection

Projection, base width, gel cohesiveness, shell texture, and shape - each parameter considered separately

  4. 4
    Surgical precision and scar control

Inframammary incision as the standard for minimal visibility and precise positioning

  5. 5
    Realistic communication of expectations

We clearly explain what can be achieved - and what cannot

  6. 6
    Monitoring results over time

Photo documentation, follow-up examinations, and long-term monitoring of every case

U Poliklinici Maletić use only MENTOR™ implants by Johnson & Johnson - a global leader in breast aesthetics with a tradition longer than 50 years.

Why did we choose one brand instead of offering multiple manufacturers? Because we know how MENTOR™ implants behave in every possible anatomy - after thousands of implantations. That predictability means a more precise plan and a safer result for you.

What a MENTOR™ implant guarantees:
• European Union CE certificate and FDA approval (U.S. Food and Drug Administration)
• Cohesive silicone gel - firm, not liquid - so if the shell is damaged, the gel does not leak
• Manufacturer's lifetime warranty for replacement in case of rupture
• Every patient at Maletic Clinic receives a certificate with the type, size, and serial number of the implanted device

The implant type - round or anatomical, projection, base width, and gel cohesiveness - is selected individually during the consultation, according to your chest measurements and desired result.

PROCEDURE COURSE

What breast augmentation surgery looks like - step by step

The surgery is most often performed under general anesthesia. Duration depends on the method, anatomy, and any additional corrections.

1

Arrival and antiseptic preparation

The patient arrives fasting, after showering with a special antiseptic agent, changes into hospital clothing, and puts on anti-embolism stockings

2

Preoperative preparation

Health assessment, review of test results, and finalization of the treatment plan

3

Marking and measurement

Marking on the body and final confirmation of implant size and type - this step is crucial for symmetry and final appearance

4

Induction of anesthesia

Washing of the surgical field and sterile draping

5

Incision and pocket creation

Planned incision and creation of the implant pocket; in lipofilling, harvesting fat tissue from the donor area

6

Implant placement

The implant is positioned in the planned layer, or fat tissue is carefully injected in layers

7

Incision closure

Sutures - most often absorbable stitches that do not require removal; a drain may be placed according to protocol

8

Compression bra and recovery

The patient receives a special bra. A single room, 24-hour care - discharge home with written instructions and a follow-up appointment

We educate all patients on how to properly massage the breasts after the procedure.

PREOPERATIVE PREPARATION

Preparation for breast augmentation - what should you do before the procedure?

Good preparation is not a bureaucratic formality - it is part of the procedure. Preparation reduces risks and makes recovery easier.

Standard preoperative preparation includes blood laboratory tests, ECG, and an anesthesiologist examination. Depending on age and indication, breast imaging may also be required - ultrasound, mammography, or MRI. Health status, medication use, and possible allergies are also checked.

The Maletic Clinic advantage

Preparation on the day of the procedure

Basic preoperative preparation - laboratory tests, ECG, and anesthesiologist examination - can be performed with us on the day of the procedure.

There is no need for separate visits to different institutions.

Breast augmentation consultations at Maletic Clinic are not a "size offer" - they are a medical examination and planning. During the examination, we assess chest width, symmetry, nipple position, skin quality, the amount of existing tissue, and your goals and lifestyle.

Finally - a surgeon who analyzes your body before recommending an implant.

How does size selection work at Maletic Clinic?

During the consultation, you put on a special bra into which we place implants of different sizes. You physically feel the weight, see the volume on your own body, and make the decision together with the surgeon.

This is not a digital simulation on a screen - it is a real feeling that helps you understand what each implant size means specifically for your build.

After the trial process and the doctor's analysis of proportions and dimensions, implant size and shape are selected by agreement between the surgeon and the patient. Even if you have specific wishes - "natural and discreet" or "more pronounced cleavage" - the goal is to translate that wish into a realistic, safe plan.

Without a generic "one size fits all" approach.

Operating room equipment - why it matters in implant surgery

In breast augmentation, a foreign body - a silicone implant - is placed in the body. In this context, air quality in the operating room directly affects the risk of infection.

Maletic Clinic uses state-of-the-art operating rooms with air recuperators that continuously purify the air and return maximally purified air into the room. This system significantly reduces the concentration of microorganisms in the room during the procedure.

Even if at other consultations you received only information about the "type of implant and price" - ask about the operating room. The equipment in which the procedure is performed is just as important as the surgeon's hands.

What a patient at Maletic Clinic receives with the procedure:
• Operating room with air recuperators to minimize infection risk
• Stay in a single room with a bathroom
• 24-hour medical care after the procedure
• Special compression bra
• Written instructions for the postoperative period
• Education on proper breast massage for an optimal result

POSTOPERATIVE PERIOD

Recovery after breast augmentation - what to expect in the first 7 days, 2 weeks, and 6 weeks

Recovery intensity depends on implant placement and the individual pain threshold. If needed, pain medication is recommended in consultation with the doctor.

Recovery timeline after breast augmentation:

Time frame What to expect Restrictions and recommendations
First 7 days Pressure, swelling, sensitivity - the most intense period; return to office work Rest and avoidance of physical exertion
10-14 days Swelling decreases; gradual return to all activities Avoid lifting heavy loads and intense activity
3-6 weeks The tissue goes through the healing phase; the implant settles into the pocket Wearing a compression bra - stabilizes the tissue and supports implant position
After 6 weeks The breast gains a more natural drop and softness; the final appearance forms gradually Full return to all activities according to the surgeon's instructions

The compression bra stabilizes the tissue and supports implant position while the body goes through the healing phase.

SAFETY AND RISKS

Risks and possible complications - what is important to know before deciding

Every surgical procedure carries certain risks - including risks of anesthesia, bleeding, and infection. In breast augmentation with implants, there are also specific complications.

Specific implant complications

Capsular contracture, changes in sensation, implant displacement or rotation, visible edges (rippling), and, in the long term, possible rupture implantata.

BIA-ALCL and "breast implant illness"

Patients increasingly ask about BIA-ALCL - a rare disease associated primarily with certain types of textured implants - and about "breast implant illness".

How Maletic Clinic minimizes risks

  • Adequate preoperative preparation and health assessment
  • Only certified MENTOR™ implants with warranty
  • Air recuperators in the operating room filter and return maximally purified air, reducing infection risk
  • Single rooms with 24-hour medical care
  • Experience of the surgeon and the entire medical team since 1998

RESULTS AND FOLLOW-UP

Breast augmentation results - what you can expect and when

The breast augmentation result is visible immediately in terms of volume. The final appearance forms over several weeks – swelling gradually decreases, the tissue settles, and the breast gains a more natural drop and softness.

The implant settles, the tissue adapts, and the final appearance forms gradually. MENTOR™ cohesive gel retains its shape and, in case of rupture, does not spread through the breast tissue.

The appearance in the first few weeks is not the final result. That is why follow-up examinations and long-term monitoring are an integral part of the protocol at Maletic Clinic.

SELECTION CRITERIA

Criteria for choosing a clinic for breast augmentation

If you are considering breast augmentation, the most important thing is not to be guided only by photos from the internet or price “promotions”. Be guided by planning quality, safety, and transparency.

Even if you have already spoken with another clinic or received an offer that was not based on a detailed analysis of your body, it is worth seeking an opinion where planning comes before sales.

Five criteria for choosing a clinic for breast augmentation:

Criterion What to look for
Warning sign
Surgeon's experience A surgeon who knows and applies all augmentation methods and techniques Only one routine technique for all patients
Equipment Air recuperators in the operating room, single rooms, 24-hour care No information about operating room equipment
Individual approach Analysis of the chest, breast base, and tissue before every recommendation Immediate offer of a "standard size" without examination
Transparency about implants Clear information: manufacturer, certificates (CE, FDA), warranty, registration No information about implant brand or certificates
Documenting results Photo documentation before surgery and at follow-ups No result-monitoring protocol

SELECTION CRITERIA

Frequently asked questions about breast augmentation

Which method is better: implants or fat tissue?

For each patient, we individually choose the method that is optimal for her. Sometimes this means implants, sometimes lipofilling, and sometimes a combination of both methods. After a detailed examination and discussion of wishes, we decide together.

How long does recovery take and when can I return to work?

In the first few days, you may feel pressure, discomfort, and swelling. Heavier physical activity should be avoided for 10-14 days. Return to office work is usually possible as early as 2-3 days after the procedure.

Will I be able to breastfeed after surgery?

Breast augmentation surgery does not disturb the glandular breast tissue - breastfeeding is generally possible. We recommend discussing this openly with the surgeon during the consultation.

What kind of scar can I expect and how should I care for it?

With implant placement through an inframammary incision, the scar is in the fold beneath the breast - when standing, it is usually not visible. It is several centimeters long, depending on implant size. With lipofilling, scars are only a few millimeters long. We provide written care instructions; if needed, laser treatment can also be applied.

When can I exercise again?

As a rule, it is possible to resume exercise 10-14 days after the procedure, gradually and according to the surgeon's instructions.

How long do implants last and do they need to be replaced?

The MENTOR™ implants we use are of top quality - they do not need to be routinely replaced. Replacement is recommended only in case of complications. The manufacturer covers lifetime free replacement in case of rupture.

What if I want a natural look but have little of my own tissue?

A natural appearance can also be achieved with implants - with appropriate selection of size, shape, and placement. This decision is always individual and depends on your anatomy.

Can an implant rupture? What happens then?

MENTOR™ implants are made of cohesive gel that does not leak through breast tissue in case of rupture. In case of rupture, Johnson & Johnson covers lifetime free replacement.

Do implants have to be replaced after 10 years?

The implants we use are of top quality - routine replacement after 10 years is not necessary.

Will I be able to have an ultrasound or mammogram if I have implants?

Yes - both breast ultrasound and mammography are performed in patients with implants.
